Home health nurses and therapists spend a considerable amount of time documenting the care they are providing to a patient as well as the ongoing condition and progress of the patient. For decades this documentation was in paper form, on tiny, dense pages. Some assessments, such as a Start of Care, contain 10-20 pages to fill out during a 2 hour visit.
In an effort to digitize this experience and to efficiently develop and support the 100+ assessments, we created a framework of assessment questions to tackle the type of question formats we identified on the paper forms. Images below show a typical paper assessment and some highlights of the assessment framework solving the digitization challenge.
